Your rights as a patient, how we handle your information and how to ask for help if anything on this site is not accessible to you.
This notice describes how medical information about you may be used and disclosed and how you can get access to it. Federal HIPAA law requires us to protect the privacy of your health information, to notify you of our legal duties, and to follow the terms of the notice currently in effect.
Your rights include: getting a copy of your record and images, asking us to correct it, requesting confidential communications, asking us to limit what we share, getting a list of those with whom we have shared it, and filing a complaint if you believe your rights were violated.
